We collect payments in-full at the time of service or as prepaid packages.
We accept cash, checks with photo ID, credit cards, FSA, and HSA.
We can offer you a receipt with insurance codes that may be submitted to your insurer.

We do not imply or guarantee any insurance coverage.
All patients are responsible for knowing their own insurance coverage.
Pls call the number on the back of your insurance card for coverage details.
We no longer communicate with insurance companies or provide billing services on behalf of patients. Thank you.
